Dear Parents,

We have limited spots in Camp Rocks! this summer. We would love to have your child in our summer camp. Please fill out the following two pages by printing them out and filling them out by hand. Then simply fax the two pages to us at 702.368.0005. We will contact you and let you know if your child is enrolled. We must make sure that the instrumentation is adequate and that each band has similar age and ability level. Some students may be turned away, though we will try very hard to not do that.
